Nurse Practitioner / Physician Assistant - Palliative Care

Reports To: North Palliative Care Regional Director

Status: Hourly

FT, PT, PRN: Part time with limited benefits

Shift: Weekdays, Varied schedule

Location: Provider will travel locally to see patients in Palliative Care in the following areas - Haywood county. Mileage Reimbursement Included.

Summary

Provide palliative care consultation and treatment to patients with serious and life-limiting illnesses in their homes, skilled nursing facilities, assisted living facilities, outpatient clinic, and hospital.

Essential Duties & Responsibilities

  • Provide palliative care consultation and follow-up to patients and their involved persons. Consultations/follow up will include: education, therapeutic communication, and management of symptoms related to life-limiting conditions. Care will be performed in coordination with an interdisciplinary team, which will include: a collaborating physician, registered nurse, and social worker. Chaplain and musical therapist support is also available
  • Provide Palliative Care in patient’s homes, ALF/SNF, outpatient clinic, and hospital locations and submit mileage reimbursement monthly.
  • Participate in ongoing learning. This will include, but is not limited to: learning symptom management of complex illnesses, communication skills, and prognostication
  • Coordinate care and communicate with referral sources.
  • Maintain professional appearance and presentation.
  • Work to achieve performance goals, pursue progress toward individual development plan (IDP) and demonstrate Four Seasons values and competencies
  • Actively participate in Interdisciplinary Team meetings for development and evaluation of the clients plan of care
  • Create and maintain accurate patient records within the electronic medical record and complete per policy timeframe
  • Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
  • Serve as clinical resource for staff of Four Seasons, staff at facilities served, and providers in the community
  • Maintain current clinical education and appropriate licensure
  • Perform other duties as requested or assigned

  • Knowledge of electronic medical records systems

Minimum Qualifications

Education:

  • Masters or Doctorate Level

Experience:

  • 2 years of clinical experience as a medical provider is preferred
  • Palliative care experience is preferred

Licensures & Certifications:

  • Current NP or PA Board Approved Certification
  • Current NC State License in good standing
  • Current DEA Registration in good standing
  • Meet and maintain eligibility requirements to be appointed medical staff and qualify for privileges at partner facilities
  • Valid driver’s license and proof of automobile insurance.

  • Occasional night and weekend phone call coverage, shared among a large clinical team of providers
  • Willingness to drive a variety of terrain and miles between patient locations
