Hybrid Nurse Practitioner (NP) Multiple Lines of Business Position Overview: We are seeking a highly skilled and adaptable Nurse Practitioner to join our multidisciplinary healthcare team. This hybrid role spans multiple lines of business, providing services across Skilled Nursing Facilities (SNFs), Assisted Living Facilities (ALFs), and Transitional Care Management (TCM) for in-home and telemedicine patients. The NP will be trained in Osteopathic Manipulative Treatment (OMT) follow-up care in SNFs, oversee pain management in ALFs, and serve as the primary provider for TCM patients, ensuring comprehensive care coordination and improved patient outcomes. Key Responsibilities: SNF (OMT Follow-Up) Collaborate with Osteopathic Physicians to deliver OMT follow-up care for post-treatment evaluations. Monitor patient progress, document clinical outcomes, and adjust care plans as directed. Communicate effectively with SNF staff, therapy teams, and families regarding patient progress. ALF (Pain Management) Conduct pain assessments and develop individualized pain management plans in coordination with facility staff. Manage chronic conditions, optimize non-pharmacological interventions, and oversee medication adjustments. Serve as a liaison between residents, facility teams, and attending physicians to ensure quality pain management care. Transitional Care Management (TCM In-Home/Telemedicine) Act as the primary provider for post-acute TCM patients, including conducting in-home or telemedicine visits. Provide follow-up care post-hospital discharge to reduce readmissions and ensure continuity of care. Perform comprehensive health assessments, medication reconciliation, and care plan adjustments. Qualifications: Current and unrestricted Nurse Practitioner license (Family or Adult-Gerontology preferred). DEA registration and prescriptive authority. Minimum 2 years of clinical NP experience; post-acute or geriatric experience preferred. Strong knowledge of chronic disease management, pain management strategies, and transitional care. Excellent communication, documentation, and care coordination skills. Flexibility to travel between facilities and provide telemedicine visits as needed. Training & Support: Full training in OMT follow-up protocols provided by Osteopathic Physicians. Access to advanced pain management strategies and TCM workflows. Ongoing support from multidisciplinary teams, including physicians, therapy staff, and care coordinators. Schedule & Work Environment: Hybrid schedule with on-site rounds in SNFs/ALFs and remote telemedicine coverage. Collaborative and patient-focused environment, emphasizing holistic care and improved outcomes.
